BOOKING INSTRUCTIONS

STEP 1: NEW Clients ONLY- Complete Medical Clearance Form
  1. New Clients: Schedule an "appointment" (it does not matter the appointment time you select, this is simply so that paperwork can be triggered to you).
  2. You will need to create an account to proceed.
  3. You will be prompted to complete a health and aesthetic history form.
  4. Please EMAIL a photo of your driver's license and a clear, front-facing headshot in good lighting to hello@beautifullyyou.info
  5. Thank you for your understanding and support. We strive to provide the safest and highest quality care while maintaining compliance with Illinois Law.
  6. Proceed to step 2 immediately after you complete your forms.

Medical Intake Form

STEP 2: Book with Christina
  1. Returning Clients may skip step 1 and book directly with Christina.
  2. If you are unsure of what treatment to choose, select a complimentary 30-minute consultation with Christina.
  3. All other appointments that are not a consultation or follow-up require a $100 deposit that is applied to the overall cost of treatment.
  4. Per policy, the deposit is nonrefundable if cancellation is less than a 24-hour notice.
  5. Any large service packages will require a 30% nonrefundable deposit to book (an in-person consultation is required with Christina first, please select consultation if interested in Microneedling, Hair Restoration, or PRP Filler).
  6. New clients: be prepared to sign treatment-specific consents at the time of booking.
